You must supply the following information

Name and dosage of medication
What it is for
When it was last prescribed

You may be asked to make an appointment for review if it is more than six months since medication was prescribed.

If your prescription is due for renewal over a holiday period (Easter, Christmas, New Year etc) please place your request at least a week beforehand.
